3、投標人資格要求
投標人應具備的資格或業績:投標人資格和業績要求：

投標人需提供以下文件
The following documents must be provided
*（1）投標人須按招標文件第一冊第四章格式IV-9.資格證明文件格式的要求填寫和提交有關資料。
*（2）投標產品原產地在關境外的投標人必須是制造商；或者是制造商直接授權的代理商，代理商投標需具備以下條件：
①必須提供制造商對本次投標的唯一正式授權書（授權書必須由制造商直接授權，不允許二級授權）；授權書中應包括“制造商對產品及售后服務質量負責”的相關內容和制造商聯系人及聯系方式，并確保制造商聯系人能夠協調處理項目執行過程中的重要事項。（對于此要求，制造商也可在授權書外另行出具書面承諾/說明），否則不接受代理投標；
②投標人在當地依法登記注冊的時間距開標日滿三年，需提供營業執照等注冊登記的有效證件復印件；
③距開標日三年內，投標人須具有同類產品至少兩個不同年份的銷售業績，需提供證明其銷售業績的合同或訂單復印件（至少提供2份不同年度的合同或訂單復印件）或用戶證明，并由投標人代表簽字或蓋章。
*（3）投標產品原產地在關境內的投標人必須是制造商（不接受代理商投標）。
*（4）投標人未被中國石************有限公司投標人失信行為管理辦法》的規定，失信積分達到8分時，暫停失信行為人投標資格半年；失信積分達到9分時，暫停失信行為人投標資格一年；失信積分達到10分時，暫停失信行為人投標資格兩年。暫停期滿后恢復投標資格，有效期內的失信記******委員會現場在對應網站的查詢結果為準。

************集團公司機電辦處理結果為：不予受理、駁回、維持原評標結果）達到一次及以上的，投標將被否決。

*（8）第八章中要求的其他合格的資格證書。
*（9）投標人資質需要有放射源經銷運輸資質。投標人應提供本公司的《輻射安全許可證》，許可證中應明確有銷售和使用IV類V類放射源的資質。
*（10）投標人所供產品的制造商具有近10年來在中國境內2個及以上石油化工企業連續良好運行1年以上的應用業績。每個合同射線料位計數量不少于6臺。投標人須提供業績證明資料，未按照要求提供業績證明資料的業績為無效業績。業績證明資料為合同關鍵頁復印件【封皮、分項供貨清單，可覆蓋價格，雙方蓋章頁】，技術協議，調試報告。招標人有權赴現場復核投標人提供的投標文件的真實性，如投標人不配合現場復核，被視為無效投標。未按照要求提供業績表和業績證明資料的業績為無效業績。投標人所提供的業績證明材料有虛假內容的，將被視為無效投標，并予以拒絕。
(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
(2) The bidder whose product origin is outside the customs must be the manufacturer; Or the agent directly authorized by the manufacturer. The agent shall meet the following conditions for bidding:
① The bidder should provide a letter with sole official authorized by Manufacturer for the bid；
② the Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
③ The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (******). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) The above contents are subject to the inquiry results of the bid evaluation Committee on the corresponding website.
(5)Bidders are not included in CNPC Suppliers, contractors, service providers blacklist (Suppliers, contractors, service providers blacklist show at ******)
(6) The bid shall be rejected if the bidder puts forward invalid complaint one or more times in the bidding activities organized by China Petroleum Materials Co., Ltd. within one year before the bid opening date (including complaint rejected, overruled or judged to maintain the original bid evaluation results by the mechanical and electrical office).
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
(9) The bidder shall be qualified for distribution and transportation of radioactive sources. The Bidder shall provide the Radiation Safety License of the Company, which shall specify the qualification for selling and using Class IV and V radioactive sources.
(10) The manufacturer of the products supplied by the bidder has the application performance of continuous good operation for more than one year in two or more petrochemical enterprises in China in the past 10 years. The quantity of each contract ray level indicator shall not be less than 6 sets. The bidder must provide the performance certification data, and the performance that fails to provide the performance certification data as required is invalid.
